All organizations submitting a completed application and signed waiver and statement of understanding agree that the “12 FOR LIFE NYS” Campaign is designed to assist organizations that meet the “12 FOR LIFE NYS” requirements and that they must fundraise completely on their own for the purposes of being included in a bulk purchase of AEDs if and when they meet the $1200.00 per site goal. All agency’s have volunteered for the privilege of being able to be a part of this effort and understand that the “12 FOR LIFE NYS” campaign has no responsibility other than providing the web based fundraising tool, confirmation of the participating organization’s credentials and ultimately, the distribution of all AEDs to the successful participating organizations. These AEDs will be purchased and distributed when all of the individual sites achieve 100% of the listed goal on the main page. This goal will be the sum of all site goals, each being $1200.00 individually. Should their be any sites that have not met their goal by May 31, 2010, the total amount of all of the under achieved sites fundraising will be combined to purchase as many AED’s as possible with the funds. These AEDs will be distributed to the sites that did not meet their goal and whose name(s) were chosen by an outside organization in a public, fair and equitable manner decided solely by the NFHP B.O.D. Any and all excess revenue at the completion of the campaign will be dispersed to the National Foundation for Human Potential and The Louis J. Acompora Memorial Foundation to cover the expense of total campaign (i.e. websites, web program license fee, print, marketing, labor expenses) and if possible, cover any additional costs of site(s) that did not meet required fundraising amount. The disbursement of AED’s on or before 5/31/10 will mark the end of the “12 FOR LIFE NYS” campaign.
